What's Included in the Primary Care Membership

• Preventive visits and routine care
• Chronic disease management (diabetes, thyroid, hypertension)
• Basic acute care visits
• Care coordination and referrals

Additional Programs (Not Included)

• Medical weight loss programs
• GLP-1 medications
• Hormone optimization

Patient may still use insurance for labs, imaging, and specialist care. We're happy to help guide you on how to use your coverage alongside your membership.
